Although I would prefer not having to mention these things, I feel that it is best to be up-front with everyone. 
Therefore here is the fine print - large enough for you to read!

Payment:

I accept cash, checks, VISA and Mastercard.  Payment is expected at the time of the treatment.

On-Time Policy:

As I have set aside an hour of my time to spend with you, I will do my best to be on time for your appointment.  I would appreciate it if you could arrive a few minutes early so you can relax for a moment or two before we begin your treatment. If you are going to be a few minutes late, please phone and let me know.

Cancellation and Missed-Appointment Policy:

When you make an appointment, I set aside that time just for you.  I will give you my complete attention during our time together.  Because our time (both yours and mine) is valuable, I ask that you notify me 24 hours in advance if you will not be able to keep your appointment so that I may provide that time to another person who may be in pain or who needs to receive treatment.  Unless there is an emergency such as an illness or vehicle breakdown, I ask that you pay for all missed appointments in full.
